Communication is very important as we prepare for the upcoming summer. We require certain information and forms from our campers. Please submit the online forms below.

These forms are in PDF format. Adobe Acrobat Reader is needed to view the files, which you can download for free. If you have any problems or questions, feel free to call us at 1-866-621-CAMP. All forms should be in within 3 weeks of your hired date.  Paperwork sent to us prior to June 1st should be sent to our NY address.  After June 1st, please send to our summer address.
